IMPORTANT NOTES
· Player has 5 days to remit payment once invoice is sent. If Boot Room Soccer School, Inc. does not receive payment within allotted 5 day period, payment is considered late and will be assessed a $50 late fee.

·We also accept Visa & Mastercard. The card type, the name on the card & the ccount number, expiration date, and the 3 or 4 digit security code are needed.
· Boot Room Soccer School, Inc., reserves the right to change/adjust the fee schedule.
· All players must fill out a Boot Room Soccer School, Inc Form Package (medical waiver, cancellation policy, equipment policy, training agreement, registration) prior to starting their training program.
